Generic Drugs and Older Adults

Older adults take an average of five prescription drugs daily, representing the largest group of prescription drug users. The two most widely prescribed drugs among this age group are Lipitor [cholesterol lowering] at 4.3m daily and Plavix [blood thinner/claudication] at 1.4m daily. These are also two of the most expensive prescription drugs. Unfortunately, they are not available in generic form.

In 2010 the average brand name drug cost $198/$24 copay. Generics average $72/$6 copay. Prescription drugs are a financial hardship for many older people living on fixed incomes. Here is a listing of when the most widely prescribed drugs will be available in generics:

Lipitor late 2011
Plavix 2012
Lexapro 2012 [antidepressant]
Cymbalta 2013 [anti-inflammatory]

Why are brand name drugs so expensive? It is estimated that over a ten-year period, $802m is spent on development, research, advertising, and marketing of the brand name drug.

Are generics inferior? The FDA says no, they are "approximately the same." However, manufactures are allowed a 4% differentiation in the formula: In some cases, that is a problem.

My concerns are twofold. First, drug stores buy from different manufacturers whose formulas vary slightly. It is important to LOOK ON THE BOTTLE FOR THE NAME OF THE MANUFACTURER. It is there in small print. Check that every month and if you notice various manufacturers, tell your physician. He or she may not want that much variation in the formula depending on your overall health issues.

Second, drugs that sustain life or drugs with a narrow therapeutic range may not be appropriate in generic form. Always best to ask the physician.
